This is a reminder to schedule a physical with your primary care physician if your child will be playing a sport in the upcoming school year. Physicals will no longer be offered through the school.

A physical is REQUIRED to be completed for your son/daughter to participate in sports. A sports physical must be completed on/after April 15th, 2024, in order to count for the next school year.

Physical forms may be picked up in the school office. They can also be found online at:

A copy of this physical must be submitted to the school office once completed.

Please plan accordingly and do not wait until the last minute to schedule an appointment. Even if you are unsure if your student will be participating in a sport or not, it would be a good idea to have a physical completed and on file.
